Malaya of Ukrainian Academy of Medical Sciences" popup.description2 The study enrolled 169 patients with chronic kidney disease (CKD) (chronic glomerulonephritis - 91 and chronic pyelonephritis - 78), 86 patients with essential hypertension and 16 patients with acute coronary syndrome and 14 healthy donors. It was found that in patients with CKD the increase in serum levels of soluble VE-cadherin, the content of PAPP-A and raised number of TSEK are due to violations of intercellular interactions in the vascular endothelium and is the evidence for violations of the structural integrity of endothelial monolayers. Decreased plasma level of stable metabolites of nitric oxide was shown. Detected violations may complicate the course of kidney disease and promote the development of atherosclerotic lesions in patients with CKD. Achieved results have predictive value and can serve as a criterion for effectiveness of therapy and determine a more active use of anti-inflammatory therapy in patients with CKD. It was also found that the use of L-arginine for intravenous injection against conventional antihypertensive and renoprotektive therapy improves the NO-producing capacity of the endothelium, leads to a decrease in proteinuria and improvement in health status of patients, adjusts the functional parameters of monocytes, and diminishes heartvascular complications in these patients by reducing the deficiency of nitric oxide.
