Malaya of Ukrainian Academy of Medical Sciences" popup.description2 On the basis of 266 patients examination it was found that in patients with diabetic nephropathy (DN) combined with arterial hypertension (AH) the progressive increase in aldosterone which depends on the stage of NAM, the blood pressure and glucose levels. Progressive increase in the chymase activity against reduced activity of its inhibitors lead to further stimulation of aldosterone synthesis. On the background of increased concentrations of aldosterone found in DN, the statistically significant increase of RAI-1 have been shown, which plays a potential role in the development of glomerulosclerosis and intersticial fibrosis of kidneys. Progression of DN accompanied by a substantial increase in plasma aldosterone and RAI-1 levels. Aldosterone also stimulated the secretion of RAI-1 by leukocytes in blood of patients with DN. On the basis of these data the means for initial manifestations of diabetic nephropathy were developed as well as the new methods of the differentiated application of antihypertension, angioprotection facilities and metabolic therapy for the treatment of patients with diabetic nephropathy and hypertension were suggested.
