Methods: dopplerechocardiography (ECHO), artery duplex scan, electrocardiography (ECG), vectorcardiography, high resolution ECG, holter monitoring of ECG and arterial pressure (AP), statistical methods. Objects: hypertensive patients (EHP) (179), EHP with persistant AF (32) and permanent AF(60), EHP undergone atherotromotic IS (85) and cardiembolic IS (49), in common 490 elderly patients. Peculiarities of myocardial inhomogeinity and its relationship with heart arrhythmias in noncomlicated EH and in EH complicated by persistant and permanent AF, atherothromotic and cardioembolic IS were established. Specifics of cardiac and arteries remodeling, AP circadian rhythm disturbances was shown to be determined by character of comorbid pathology in senile EH. There were revealed relationships between heart and elastic arteries structural-functional changes and myocardial electrical inhomogeinity : registration on noninvasive markers of electrical inhomogeinity and cardiac arrhythmias appeared to be associated with more marked left ventricular hypertrophy (LVH), heart cavities enlargement, hypertrophy and rigidity of common carotids arteries. . Dissociation between ECG and ECHO LVH is accompanied by the increase of myocardial electrical inhomogeinity Gender and age-related differences for forming myocardial electrical inhomogeinity, cardiac and arteries remodeling, systolic and diastolic function of left venticular were revealed. The character and expressiveness of atherosclerotic leisures of cerebral arteries were determined.Close relationships between markers of myocardial electrical inhomogeinity, heart . structure and function, cerebral blood flow being obtained evidenced parallelism in forming heart and cerebral arteries disturbances in EH complicated by AF. Specifics of heart arrhythmias as well as conduction deterioration was shown to be related to hemispheric localization of IS. The addition of antihypoxic and antioxydant drug "Mexicor" to standard therapy demonstrated favorable influence on circadian rhythm of AP, the decrease of ventricular arrhythmias , the improvement of cerebral circulation in patient with AF undergone cardioembolic IS.
