Objective - based on the study of metabolic risk factors (dyslipidemia, hiperhomotsysteyinemiya (GHz), "C"-reactive protein (CRP)) of cardiovascular disease in patients with RA, their connection with disease, develop approaches to identify predictors that damage the cardiovascular system and develop recommendations for rehabilitation of disabled persons in this category of patients. Methods - clinical, radiological, biochemical, HRP, instrumental, statistical. The first time comprehensively studied metabolic risk factors for cardiovascular disease in patients with RA in their relationship with features of the underlying disease and the condition of the cardiovascular system, developed by individual and typical rehabilitation program of invalids after RA. It is shown that in patients with RA has been a violation of endothelial function, which is combined with thickening of intima-media complex of the common carotid arteries and increase the number of atherosclerotic plaques. It has been found, that morphological and functional condition of myocardium in patients with RA is characterized by increasing of the myocardial mass of left ventricular and its remodeling. For the first time in patients with RA demonstrated strong direct correlation between left ventricular mass and level of homocysteine in the blood. Its is shown that myocardium hypertrophy and left ventricular diastolic dysfunction is frequently encountered among patients with high СRP and tumor necrosis factor-alpha (TNF-?), a high value on the disease activity scale (DAS) and functional failure patients (HAQ). The high activity of inflammation (increased amount of blood СRP, TNF-?), GHz and lower cholesterol high density lipoprotein (LDL HDL) levels - are the predictors of lesion of the cardiovascular system in patients with RA. The main causes of disability in patients with RA are dysfunction of the joints and moderate and / or high activity of inflammation (61.6% and 60.9%, respectively) and in 32.2% cases of severe cardio-vascular pathology. Among the disabled, the frequency of cardiovascular disease by more than 7.5 times higher than among patients with RA not recognized disability, in 57.5% of cases it is combined with a severe course of disease and in 60.3% of the dysfunction of the joints II and III degree.
