Objective of study. Increase the effectiveness of treatment of gynecological cancer patients due to the development and optimization of surgical techniques. New surgical techniques for the treatment of gynecological cancer patients have been developed and introduced into clinical practice, namely: radical abdominal trachelectomia in cervical cancer (CC) patients of childbearing age, combined surgery in patients with locally advanced cervical cancer, combined surgery in patients with vulva cancer (VC), hysteroresection of endometrium in patients with primary uterine body cancer (UBC), organ saving surgical interventions in patients with early forms of ovarian malignancies (OM) and combined surgery in patients with OM of II-IV stages, including the retroperitoneal hysterectomy with pelvic peritonectomia. It has been shown that in patients with early OM in childbearing age organ saving treatment may be carried out: 3-year survival is not different from survival after standard operations (panhysterectomia, omentectomia) - (89,2 ± 9,9)% and (86,3 ± 5.5)%, respectively (p > 0.05). In all patients of the main group menstrual function has been preserved, 8 patients after the treatment gave birth to babies. Radical abdominal trachelectomia allows preserving reproductive and menstrual function in women of childbearing age, improves the emotional status and quality of life of patients with cervical cancer. Combined surgery in patients with advanced VC is an effective and efficient method of treatment, partial resection of adjacent organ does not significantly affect the duration of surgery, blood loss, intra-and postoperative complications, 3-year survival of patients with VC (T3) after combined operations was (63,7 ± 8,3)%, 5-year survival was (44,0 ± 9,4)%. Extended and combined primary cytoreductive surgery in patients with advanced OM 6-fold increases the number of optimal operations (residual tumor macroscopically unavailable) from 9.9% (in standard surgery) to 59.3% and 41.3% respectively (p <0.05). Optimal cytoreductive operation results in 5-year survival rate in (41,4 ± 7,3)%, suboptimal (residual tumor <1 cm) - in (14,7 ± 2,1)%, suboptimal (residual tumor > 1 cm) - in (7,5 ± 4,3)% of patients. Introduction of research results into clinical practice of medical institutions of Ukraine may bring annual economic impact of more than 27,950 thousand hryvnas.
