Purpose of work - to promote efficiency of head and neck cancer patients' treatment by development and improvement of combined, extended and reconstructive plastic operations. Research methods: clinical, laboratorial, roentgenological, histological and statistical. Results: A working classification of postoperative defects in head and neck including location and extent of resection was developed. It allows to plan the reconstruction with the varied choice of the optimal autotransplantat. Found that the use of autografting vascularized tissue for reconstruction of complex oropharynx defects in patients with locally advanced oral cancer in 21.9% allows to decrease the rate of salivary fistulas compared with traditional methods of plastic. Found that the most effective method of restoring large postoperative defects of the skin and soft tissues of the face and neck, and mucous oropharynx area is the reconstruction of skin-muscle flap including the large breast muscle on the vascular pedicle.
