Study Group n = 38 infants with prenatally diagnosed critical CHD, which operated in the first hours of life using autologous cord blood during surgery 2. Control group n = 60 infants with postnatally diagnosed critical CHD, which operated using donor blood during surgery Objective: justify immunological effectiveness of autologous cord blood in newborns with congenital heart disease by identifying features production of proinflammatory and anti-inflammatory cytokines (TNF-?, IL-1?, IL-6, IL-8, IL-10), HIF and anti- Hsp60 antibodies in the serum. Methods: immunobiological investigation of newborns with CHD by identifying features production of proinflammatory and anti-inflammatory cytokines (TNF-?, IL-1?, IL-6, IL-8, IL-10) and anti-Hsp60 antibodies before and after surgery were measured using advanced technologies (different versions ELISA analysis, affine chromatography). For the first time in Ukraine and the world was proposed using of autologous cord blood for open cardiac surgery in newborns with prenatally diagnosed CHD. From 2010 to 2012 year, 98 infants with prenatally diagnosed critical CHD were operated. In 38 patients we avoided using homologous blood products perioperatively. We suggest that autologous cord blood owing to fetal hemoglobin is able to provide adequate oxygen delivery in a more severe hemodilution during CPB. Umbilical cord blood is a safe and effective alternative to homologous blood in neonatal cardiac surgery. It was shown that autologous cord blood contributes to adaptive immune system resource after open heart surgery. Results of research presented in 17 articles, 22 abstracts, 3 patents for invention, 2-scientific works, 22 presentations at conferences, including foreign, developed and introduced into clinical practice protocol "Pregnancy and delivery in pregnant women with prenatally diagnosed congenital heart disease".
