G. Yanovsky popup.description2 Object: 487 patients with multidrug resistant tuberculosis (MDR TB) of the lungs. Objective: to improve the treatment results of patients with MDR TB by developing optimal combinations and the number of anti-TB drugs in the regimens of chemotherapy (CT), and set the length of the intensive and continuation phases of CT. Methods: clinical, radiological, laboratory, microbiological. Among patients with advanced MDR TB extensively drug-resistant tuberculosis (XDR TB) occurs in 22,4 % of cases, mainly in patients treated with anti-TB drugs of I-II line. Risk of developing XDR TB have the patients, who were treated in the past with anti-TB drugs of I-II line, with bilateral infiltrative focal lesions in the lungs and large cavities, anti-TB drugs cancellation due to poor tolerability. The effectiveness of treatment is reduced in XDR, low adherernce to treatment, case of presence of 2 and more cavities of various sizes, the extensive and symptomatic lung disease, the use in the past regimes with anti-TB drugs of I-II line; F drug-resistance of MBT; use at the initial phase of chemotherapy 3 anti-TB drugs which have retained sensitivity . Developed individual chemotherapy regimens requiring appointment of 1-3 anti-TB drugs with weak activity to which MBT is resistant, in addition to 3-5 anti-TB drugs to which MBT is susceptible. It was established efficacy and tolerability of the developed regimens for XDR, which allows to achieve sputum conversion in 52,9 % patients and improve the treatment results on 12,9 %.
