Purpose of research - to improve the efficacy of synchronic chemoradiation therapy for non-small-cell lung cancer by means of working out new technologies of application of highly energetic photon radiation and antitumor drugs with radiosensitizing effect based on determining the mechanisms of radiation influence on induction of ceramide-mediated tumor apoptosis. Methods of research: biochemical, electron microscopy, morphology, clinical instrumental, radionuclide, dosimetry, statistical analysis. Theoretical and practical results - the obtained findings expand our understanding of the mechanisms of accumulation of the amount of proapoptotic lipid, ceramide in Guerin’s carcinoma, namely by means of synthesis de novo and hydrolysis of sphingomyelin at combined action of high-energy photon radiation and Etoposide. It was determined that in the majority of patients with non-small-cell the level of CYFRA 21-1 reduced 2.6 times against a background of the tumor regression by over 50% after chemoradiation treatment with Etoposide, which can have practical significance for therapy efficacy monitoring. Novelty - For the first time it was proven that combined action of high-energy photon radiation and Etoposide resulted in activation of acid sphingomyelinase and accumulation of ceramide in the blood. Efficacy of introduction - a method of monitoring efficacy of chemoradiation therapy for non-small-cell lung cancer allowing to choose the adequate irradiation modes promoting relapse-free period and survival prolongation was suggested.
