Aim of study: optimization of diagnostics, treatment of IBD and IBS based on pathogenetic mechanisms. Results: different signs of CTD were revealed in all IBD and IBS patients. 76,3% patients with IBD had internal signs of CTG in gastroenterological tract that lead to more severe course of disease. IBD patients had anemia, leucocytosis which correlated with extraintestinal manifestation of IBD. Inflammatory process influent on changes of mucus barrier and severity of IBD. In patients with IBS was normal endoscopical picture. At the same time 45(37,5%) IBS patients had minimal changes. All IBD patients had endoscopical changes such as decreasing of vascular pattern, erosions, ulceration, fibrin, putus, contact bleeding, pseudopolyposis. Intensiveness of endoscopy changes correlated with enlargement of IBD, but not wuth extraintestinal manifestation. Histological changes in IBD characterized by decreasing of mucus layer, goblet cells and inflammatory infiltration. Intensiveness of morphological ganger correlated with severity and activity of IBD.
