Object of investigation: development of hypoxic and post-infarct myocardial remodeling in rats and humans, its molecular and genetic mechanisms. Aim: to investigate the molecular mechanisms of cytoprotection and hypertrophy in hypoxic remodeling, their interrelation and regulation at different modes of high altitude hypoxia, and beta-adrenergic stimulation. Methods: physiological, molecular, morphological, biochemical, and functional at different levels of structural organization. These investigations allow for the first time characterize the sequence of cytoprotective, proliferative and antiproliferative mechanisms in each ventricle of the heart under different modes of hypoxia, and during beta-adrenergic stimulation. For the first time, the differences in protein expression in the myocardium of the ventricles are identified and characterized in different modes of hypoxia and at varying degrees of hypertrophy, the features of these mechanisms are characterized in remodeling of the right and left ventricle. It was found that the mechanisms of hypoxic remodeling include translational and post-translational activation of kinase Akt and GSK-3? and their reciprocal regulation, consistent stimulation of receptor IGF-1R/HSP60 - mediated, signaling Akt-dependent proliferative and GSK-3beta/rGSK-3beta - dependent antiproliferative mechanisms in the myocardium. The participation of translational activation of MnSOD, dystrophin and caveolin-3 in the mechanisms of cytoprotection is determined. As a result of research, molecular markers of risk for myocardial hypertrophy are identified, provided clinical recommendations are provided.
