Purpose - to define the pathogenic role of deviation in gene regulation of apoptosis and telomeric region of lymphocytes in the development of cerebrovascular disease in the late period after irradiation. Methods - clinical, psychometric, immunological, cell-molecular, cultural, statistical, analytical. Results. In the late period after irradiation at doses less than 250 mSv, was defined the decrease of gene expression regulators of apoptosis: TR53, TR53I3, FASLG, BAX and CCND1, which was accompanied by a decrease in the number of the cells in apoptosis and natural killer cells. In groups of irradiated and non-irradiated individuals with cerebrovascular disorders and cognitive disorders were found dose-dependent overexpression of the gene TERF1, reduction of relative telomere length of CD34 +38- stem cells and SD34 +38- early progenitor cells. According to psychometry data organic disorders with affective depressive and anxiety components were dominated. In liquidators in the late period after irradiation there is a clear dependence of gene expression indicators, telomere length, cognitive function to dose and age at exposure. Reduced telomere length is accompanied in the range of 200-1500 mSv with parallel decline of MMSE. At doses up to 200 mSv lack of dependence of the RTL from dose accompanied by preservation of cognitive function. In irradiated at the age of 30 defined dependence of the RTL and MMSE values to external dose. In liquidators who were exposed after the age of 30 years, this effect does not occur.
