It is proved that the activation of systemic low-grade inflammation is a characteristic of essential hypertension (EH), as evidenced by a significant increase of inflammatory marker C-reactive protein and inflammatory mediators blood levels in EH patients, it independent of the classical cardiovascular risk factors; and the availability of correlations between the indicators of the systemic inflammation, - on the one hand and the level of systolic and pulse - blood pressure on the other. The presence and degree of systemic inflammation, oxidative stress (especially its protein component), increased atherogenic blood potential, systemic endothelial dysfunction, insulin resistance and the state of purine metabolism (severity of uricemia) are associated with hypertensive organ damage (heart, blood vessels, kidneys). In hypertensive patients carotid atherosclerosis is associated with an increase of pro-inflammatory blood activity, microalbuminuria, low high-density cholesterol, uricemia, insulin resistance and HbA1c level, regardless of the classical cardiovascular risk factors. Shown that the use of renin-angiotensin blockers (losartan, valsartan and perindopril) in hypertensive patients reduces the activity of systemic inflammation. Losartan and valsartan promote regression of left ventricular hypertrophy, microalbuminuria reduction, improved insulin sensitivity, regardless of blood pressure decrease.
