Aim: To define patterns of disabling psychoneurological disorders in the Chornobyl clean-up workers depending on the effects of radiation and non-radiation factors, to argue therapeutic and preventive measures to improve the survivors' health. Methods: clinical, epidemiological, mathematical-and-statistical. Results: According to the cohort epidemiological study on the data of SRU and CER (68414 and 3870 clean-up workers with whole body external doses), there was found an increase of acute and chronic cerebrovascular pathology as well as mental disorders due to organic brain damage with the highest incidence observed 12-20 years after the Chornobyl accident. There were established statistically significant radiation risks of psychotic and non-psychotic disorders caused by organic brain damage, acute and chronic cerebrovascular pathology mainly at doses over 0.25-0.5 Gy, and mortality from acute and chronic cerebrovascular diseases in clean-up workers exposed at the age of 40. Statistically significant non-radiation risks for cerebrovascular diseases were identified. Using the results of sample clinical examination the local subregistry (427 people) was developed. Clinical features of the development and course of multiple sclerosis, consequences of acute cerebrovascular diseases, depressive and post-traumatic stress disorders on the background of comorbidity pathology were identified in clean-up workers. Prevalence of unfavourable non-radiation risk factors in the development of acute cerebral circulation impairment were investigated, the most unfavourable ones being defined.
