Subject of research: bone metabolism, metabolic, genetic, autoimmune factors violations reparative osteogenesis, treatment of bone nonunion. Objective: To study the prevalence based on the molecular-genetic, metabolic, autoimmune factors of vascular lesions in violation of reparative osteogenesis and establishing their connection with the state of bone disorders clarify the mechanisms of formation of reparative regeneration of long bones and develop evidence-based approaches to the prevention of delayed consolidation of bones and treatment of pseudarthrosis. Methods: physical, instrumental, biochemical, molecular genetics, morphological and statistical. The prevalence of vascular lesions in patients with impaired reparative osteogenesis and explained the role of the molecular-genetic, metabolic factors in the formation of reparative regeneration disorders investigated the first time in Ukraine. In experimental and clinical conditions studied relationship of hyperhomocysteinemia with morphological and metabolic bone status in violation of reparative osteogenesis. The interrelationship between bone tissue and genetically determined and acquired risk factors for vascular lesions in disorders of reparative osteogenesis was established. The metabolic and growth of the content of inflammatory mediators are more associated with the formation of false avitalnih types of joints and less expressive at rates and hyperplastic types. In genetic research has shown that violations of reparative bone formation is associated with development of adverse metabolic, inflammatory and vascular pattern, which to some extent determined by enzyme gene polymorphism exchange Hcy and nitric oxide. The differentiated approach to the diagnosis, prevention, and correction of pharmacological and surgical treatment of disorders of reparative ostenogenesis depending on the combination of metabolic and genetic factors bone nonunion.
