The aim of the work is to improve the functional results of the treatment of patients with innate cleft of upper lip and soft palate by elaboration and perfection of the methods of diagnosis of the reasons of palatopharyngeal insufficiency. The methods of the investigation: videoendoscopic, sound spectrometry; functional; clinical; laboratorial, mathematical, statistic. The children, having been operated before in regard to innate isolated and combined clefts of soft palate, were examined within the framework of the investigation. The spectrographic studies of the changes in amplitude-frequency characteristics at phonation of vowel sounds were held. On the basis of instrumental computer analysis the method of nasometric estimation of nasal speech of children was worked out. The peculiarities of the vegetative regulation in children with different forms of innate clefts of soft palate were studied; the peculiarities of bioelectric activity of brain in children with different functional results of their surgical treatment were revealed. The functional effect of uranoplastics was shown to be determined in considerable degree by the background neurologic state and the state of central nervous system of a child. At the analysis and the comparison of the data on the character of innate defect with the results of the studies the indices of the degree of the palatopharyngeal insufficiency and the mechanisms of its formation were determined. The indications for the execution of the methods of the correction of palatopharyngeal closure depending on the clinico-functional view of the defect or the deformation of soft palate in children, having been operated before in regard to innate clefts of upper lip and soft palate, were found. The new pathogenetically conditioned scheme of preoperational examination of children with clefts of upper lip and soft palate, in the basis of which the study of the adaptive reactions of the organism and the state of haemodynamics of operational field lies, was worked out. The new methods of prognostication, early diagnosis and prevention of postoperational complications of cheilo- and uranoplastics are suggested.
