We studied the causes of unsatisfactory results of surgical treatment of hiatal hernia, when only the cruroraphia was used. An analysis of the causes of unsatisfactory results formed a new diagnostic scheme of investigation of patients, developed protocol scheme selection method for suturing of hiatus depending on the hiatal surface area, developed a laparoscopic technique of the Strong operation, and defined the indications for its use. Proposed protocol scheme intraoperative diagnosis and method of choice surgical correction of a short esophagus through the laparoscopic approach in patients with hiatal hernia. It also was developed to measure the hiatus, which allows you to "true" value of the hiatus size, which were determined during the traditional antireflux surgery laparotomy access with the fingers of the surgeon. Using the developed original techniques has improved the immediate and long-term results of surgical treatment of patients by reducing the number of patients with recurrent hiatal hernia, and improve the quality of life of operated patients.
