The thesis considers the issue of increasing the efficacy of surgical treatment of neovascular glaucoma (NVG) of post-thrombotic and diabetic genesis by means of combined application of viscoelastic and episclera excision in the area of surgery on the basis of pathology and experimental studies of possibilities to reduce excessive proliferation of connective tissue cells and to prolong the hypotensive effect. The pathomorphological data (56 eyeballs) show that multiple combined microthrombosis of all eye vascular structures (87,0 %) is dominant with NVG of postocclusal genesis, hemorrhagic manifestations (65,2 %) are prevalent with NVG of diabetic genesis, and episclera is the source of scleral uveal adhesions. The experiments prove that viscoelastics keeps intrascleral channel and suprachoroidal space expanded, and episclera flap excision provides episclera thinning in the surgery area. Viscocycloiridoretraction with episclera excision (VCIRE) surgery hypotensive efficacy within three years is 42,9%. The offered way is the most effective for patients with postthrombotic NVG.
