Aim of the investigations: To develop the system of quality assessment based on morphological and functional criteria in case of human spermatogenesis pathology and to suggest the model for application of selected cells for in vitro fertilization.Theoretical and practical results: IMSI (intracytoplasmic morphologically selected sperm injection) method was developed to improve effectiveness of male factor infertility treatment. IMSI based on real-time motile sperm examination and selection for in vitro fertilization under magnification up to x6300. Analysis of the generated data proved effectiveness of morphofunctional sperm selection under x6300 magnification for patients with teratozoospermia and oligozoospermia. Effectiveness of sperm selection under x6300 magnification was not proved for males with asthenozoospermia. Morphofunctional sperm selection allows significantly increase blastulation rate. Correlation between sperm morphology assessment under x6300 magnification and Kruger's strict criteria was determined. Morphofunctional sperm selection could be used to improve efficiency of infertility treatment. Sperm selection under x6300 magnification could prevent vertical transfer of existent and de novo formed mutations by rigorous selection of the sperm for fertilization in vitro.
