Goal - develop optimum diagnostic algorisms and improve clinical protocols for treating acute TBI. Research methods - meta-analysis of literature, analysis of primary medical documentation, expert appraisal of the medical care quality, clinical and neurological examination (including oto-neurological), brain imaging diagnostic methods: pain craniography, spot-film radiography, computer tomography, spiral computer tomography, spiral computer-tomographic cysternography, magnetic resonance imaging. Scientific novelty - the effectiveness of modern TBI diagnostic and treatment methods was studied, and appropriate changes were made in the algorithms, standards, clinical protocols of medical care for acute TBI patients in comparison with evidence-based medicine data. Based on the research findings, the following changes in the clinical protocols for acute TBI patients were proposed: (1) standards of examination of brain damage patients at admission, (2) an algorithm of medical care provision to severe TBI patients, (3) an algorithm of medical care for patients with posttraumatic liquorrhea, (4) indicators of the quality of medical care for TBI patients, (5) using local fibrinolys in patients with traumatic intracranial hemorrhage as a selection method. In the course of the research methodological recommendations "Modern principles of forensic medical examination of patients with brain concussion" were developed and approved, a training and methodological manual "Traumatic brain injury: modern principles of emergency care" was published, a system of expert appraisal of medical care quality using the "Map of expert appraisal of the quality of medical care provided to acute TBI patients" was designed. It is proposed to introduce the developed materials into the practice of neurosurgery, neurotraumatology, otolaryngology and neurology departments. The introduction of the proposed changes based on the research results into diagnostic algorithms and medical care protocols for TBI patients will make it possible to improve treatment outcomes and TBI patients' quality of life.
