Object of research: a method/technique of surgical treatment of patients with intracerebral subtentorial tumours of different grades of malignity. Objective: improvement of outcomes after surgical treatment of patients with intracerebral subtentorial tumours. Methods: morphological, cliniconeurologic, radiologic, histologic, statistical. The peculiarities of the tumour structure and the cell structure, the vascular walls, the cell microenvironment in tumours of different histologic variants and degrees of anaplasia have been studied. The retrospective and follow-up analysis of 929 clinical cases of patients with intracerebral subtentorial tumours has been carried out. Structural and molecular peculiarities of intracerebral subtentorial tumours of different grades of malignity have been studied. The characteristic features of medical pathomorphism in patients with cerebellar medulloblastoma have been studied. Cytostatic and cytodestructive changes in tumour tissues were observed in 76% of the patients during the repeated operations. The characteristics of tumour regrowth and metastasis depending on their histostuctures, the patients' age, radicality of surgery, the tumour size and adjuvant therapy, have been determined. On the basis of clinicomorphological comparisons, the histobiological properties of intracerebral subtentorial tumours have been established, which enables us to determine the surgical tactics and the extent of tumour removal in case of their prolonged growth. The practical value of the results obtained lies in the establishment of the optimal treatment strategy in patients with intracerebral subtentorial tumours in case of their regrowth.
