Aim: to determine the optimal chemotherapy regimens for patients with MDR TB depending on the number, amount of anti-TB drugs at the treatment regimen, the length of intensive and continuation phases of chemotherapy, which will increase the treatment efficasy and save public funds. Methods: clinical, radiological, laboratory, microbiological. In patients with MDR TB, XDR TB were determined in 25,2 % of cases, and resistance to fluoroquinolone (without XDR TB ) - in 10,4 % of cases, resistance to injectable anti-TB drugs - in 21,0 % of cases, mostly in previously treated cases of I-II lines anti-TB drugs. Risk factors for the development of XDR TB or fluoroquinolone resistance or resistance to injectable anti-TB drugs were established: using more than 2 years I-II lines anti-TB drugs, large and/or multiple cavities, the prevalence of infiltrative-focal changes, the application for the previous courses of fluoroquinolones and injectable anti-TB drugs more than 1 month , the presence of side effects to anti-TB drugs and excluding them due to this side effects, proved MDR contact. The treatment efficasy in patients with different resistance to anti-TB drugs increased in those patients, who used I-II lines anti-TB drugs in the past. Individualized regimens for patients with XDR TB were developed: additional use of linezolid to 5 anti-TB drugs I-II lines, compared with regimes which used only 5 anti-TB drugs I-II lines without linezolid. Established treatmet efficasy and side effects of this regimens, which allows to reseive sputum and culture conversion in 93,3% of patients, and to increase the treatment efficasy of 41,5 %.
