Aim - to improve differential diagnosis of mediastinal lympadenopathy of various etiologies, to create a new method and algorithm for the differential diagnosis of mediastinal lympadenopathy. Methods - clinical, cytological, microbiological, pathomorphological, statistical. It has been established, that in the structure of ethiologic factors of mediastinal lympadenopathy the most frequent are sarcoidosis ((47,1 ± 1,9) %), tuberculosis ((17,9 ± 1,5) %) and malignancy ((15,3 ± 1,4) %). Informative of radiologic method for diagnosing the etiology of the mediastinal lymphadenopathy is 59,8 %. The level of discrepancy between previous clinical and pathomorphological diagnosis was (64,3 ± 4,8) %. It has been worked out 4 methods that allow to increase the informative of diagnostics and treatment efficiency.
