Objective: to increase the effectiveness of combined treatment of patients with ovarian cancer by optimizing the neoadjuvant chemotherapy regimens. Results and novelty: For the first time it's been established that the initial level of the tumor marker CA-125 is an important indicator of tumor aggressiveness and tumor process prevalence at its level of >200 U/ml, as well as the growth of the marker after the cycles of neoadjuvant chemotherapy, or the presence of poorly differentiated or undifferentiated ovarian tumors requires an amplification of the combined chemotherapy regimen. For the first time the quantity of cycles of neoadjuvant chemotherapy is substantiated, which was confirmed by morphological studies. Neoadjuvant chemotherapy reduces the total number of leukocytes, neutrophil in peripheral blood, hemoglobin levels, the absolute amount of CD20+, the content of IL-6, IL-10 and IL-1 . Established negative correlation of IL-6 and IL-10 with the relative amount of lymphocytes confirms the significant role of these cytokines in the suppression of antitumor immune defenses by raising lymphocyte apoptosis. First developed measures to improve the results of combined treatment of advanced ovarian cancer using 3 - 4 cycles of preoperative chemotherapy, which increases the median of remission and reduces the frequency of recurrence of the disease in the first two years. Optimal methods for monitoring the effectiveness of neoadjuvant chemotherapy are the level assessment of the tumor marker CA-125 and an ultrasound during the treatment, which allow making an objective prognosis to determine the possible timing and volume of surgical intervention.
