Purpose of study: To improve efficiency of synchronous radiochemotherapy for non-small-cell lung cancer by developing new technologies using high-energy photon radiation and anticancer drugs with radiosensitizing effect based on the mechanisms of induction of chemoradiation effect on ceramide-mediated apoptosis in tumors. Methods and equipment: Clinical and instrumental , biochemical, electron microscopy, morphological, radionuclide, dosimetry.Theoretical and practical results: Informativity of CYFRA 21-1 tumor marker for monitoring the effectiveness of chemoradiotherapy was determined, the mechanisms of combined effects of high-energy photon radiation and etoposide on lipid accumulation of proapoptotic ceramide synthesis and increase in Guerin's carcinoma of sphyngozin against a background of activation of apoptosis was proven experimentally and confirmed by morphological studies. Novelty: For the first time, common mechanisms of action of high-energy photon radiation and anticancer drug etoposide in induction of apoptosis in tumor ceramide pathway was shown . Effectiveness of introduction: It can increase the objective response rate to 73.7 %, increase one-year survival to 70.0 %. Area of application - Radiotherapy and oncology.
