The purpose of the study: To define the most efficient and cost-effective diagnostic methods for early detection of relapses, regional and distant metastases of differentiated thyroid cancer in the stage of post-treatment monitoring. Methods and equipment: Clinical, laboratory (thyroglobulin content -TG and antibodies to it), ultrasound, radionuclide, cytological, histological, x-ray, computed tomography. Ultrasound unit, dosimeter-radiometer, gamma-camera, x-ray unit, CT scanner. Theoretical and practical Results: The diagnostic value of immunometric and radiation methods in detection of relapses and metastases of thyroid cancer at the stage of post-treatment observation was determined. It was established that thyroglobulin level is the most sensitive diagnostic test for detection of both early and late relapses in patients with thyroid cancer. It was established that prognostic indicator of prolongation of disease is thyroglobulin dynamics. Sensitivity (96.6%) and specificity (100%) of imunometric method was determined in detection of recurrence and / or metastases of thyroid cancer. Novelty: For the first time an optimal set of radiation diagnosis methods was determined for recurrent and / or metastatic differentiated thyroid cancer: ultrasound investigation of neck, x-ray and / or computed tomography of the chest, whole-body scintigraphy after treatment with 131I, using 99mTc-(V)DMSA, which can detect not only local metastatic foci but also distant metastases in the lungs.
