Aim - determine the mechanisms of post-traumatic osteopenia based on studies of reparative osteogenesis, bone mineral density and ground of medical correction methods. Methods - morphological, bone densitometry, radiological, biochemical, cell cultivation, statistical analysis. Equipment - light microscope "AxioStar" (Carl Zeiss), bone densitometer "Explorer QDR W" (Hologic), centrifuge OPN-3М, СО2- incubator "Inco 2 108" (Memmert), thermostat ТS-80, microtom "Reichert", eyepiece micrometer МОВ-1-16x, transmission electron microscope EМV-100BR, analyser of electrolytes АEК-01. 724 patients (103 men and 621 women) with fractures in past history were investigated. Osteoporosis and osteopenia were detected in 69.0 % men and 79.4 % women which was exceeded the population data (30 %). Correlation dependence between bone mineral density, body mass index and age in women with fractures of long bone in past history was established. First find, that fractures in children and adolescent (till 26 years) and in period of "stable state" of bone (till 40 years) do not promote posttraumatic osteopenia in long term. A retrospective analysis of the treatment results of 780 patients with diaphyseal and metaphyseal fractures was shown that unsatisfactory treatment of diaphyseal fractures associated with technical and biological factors, including of bone quality disturbance. During experimental study on the rats the state of bone after femur trauma was examined. Osteopenia manifestations detected in distal and proximal zones from injury. Stimulation of bone regeneration, increase of bone mass and osteointegration of trabecular bone with a cortex were observed in case of treatment of animals with alfacalcidol and IGF-1. The method of prognostic evaluation of reparative osteogenesis was developed. Alfacalcidol treatment of 35 patients with disorders of reparative osteogenesis was efficient and helped to improve bone mineral density. Some mechanisms of posttraumatic osteopenia were disclosed. Obtained results allow increase quality of diagnosticis of patients with traumatic injuries of long bones due to introduction of bone densitometry and prediction bone repair with use created method of prognostic evaluation of reparative osteogenesis which enable timely correction of bone mineral density and improving quality of treatment of patients.
