Goal of study: to identify the main factors of radiation and non-radiation nature that influence the course of CAD, taking into account the rs966221 polymorphism of the gene of phosphodiesterase 4D (PDE4D), circulating endothelial progenitor cells (CD31 CD34) and changes in the structural and functional state of myocardium in liquidators. Methods of study: clinical examination, polymerase chain reaction restriction digestion reaction products immunefluorescent method, ECG, stress test, ECG monitoring, echo dopplercardiography, statistical analysis. Results: representative group of Chernobyl liquidators included 307 people was formed. It was not defined logical features of structural and functional parameters of myocardial changes in clean up workers compared to non-irradiated persons. This fact was pointed out that the effect of irradiation in clean up workers on the functional state of the myocardium should be considered in combination with the effect of other risk factors. Stenocardia functional class, stage of heart failure, myocardial infarction in the past, ventricular extrasystoles, and His bundle branch block, determined CAD clinical course. A statistically significant effect on the course of CAD had association of risk factors as patient's age, smoking, hypertension, diabetes and cerebrovascular accident in history. Between features determined CAD course and factors that influenced him that are a direct proportional linear relationship. Among the liquidators of the Chernobyl accident with CAD, the frequency of CC genotype is significantly reduced but TT genotype is increased, compared with patients without CAD. It was determined that patients with the TT genotype was shown earlier onset and more severe CAD course. It is established that the circulating pool of CD34-positive cells increased in liquidators with CAD. Liquidators of 1986 demonstrated the most hematopoietic CD34 (+) stem cells in peripheral blood. Reduced CD34 (+) stem cells in peripheral blood may be associated with adverse clinical course of CAD. Quantitative parameters of circulating endothelial progenitor cells CD34 (+) CD31 (+) did not differ significantly in the study groups.
