The aim was to investigate the relationship between many year dynamics of target organs condition with metabolic and proinflamatory risk factors, influence of antihypertensive drugs on renal function and risk factors.Methods of investigation - general clinical, biochemical, echocardiography, 24-hour ambulatory blood pressure monitoring, duplex scanning of neck vessels. We revealed that EH patients target organs (heart, vessels, kidneys) progression is connected with poor blood pressure control, system inflammation activity, impairment of carbohydrate, protein and purine metabolism. Angiotensin-II receptor blockader losartan, angiotensinconvertingenzyme inhibitor perindopril, calcium channel antagonist lercanidipine in EH patients with hypertensive nephropathy produced effective blood pressure decrease, kidney function amelioration (serum creatinine decrease and increase of its clearance, microalbuminuria decrease). Losartan and perindopril induced decline of systemic inflammation activity and produced uricosuric effect.Key words: essential hypertension, target organs damage dynamics, left ventricule of the heart, carotid arteries, kidney, risk factors.
