Report on scientific study (final): 93 p., 4 fig., 184 ref. Object of study: manifestations of upper and lower extremity peripheral nerve blockades. Study aim: to increase efficiency and safety of intra- and postoperative regional anesthesia in orthopedics, traumatology, and other fields. Study methods: anatomical, radiological, anthropometric, clinical, thermometric, algesoimetric, biochemical, psychometric, pharmacokinetic, ultrasonographic. Effectiveness of infra-red thermometry as predictor of successful blockade, the most rapid onset of sensory, motor, and sympathetic block after supraclavicular blockade, and the best m. trapezius anesthesia after interscalene blockade were proved. The latter also produced the most respiratory side-effects and the highest plasma levels of lidocaine. No respiratory side-effects were revealed after coracoid infraclavicular blockade. Biochemical stress-markers (glycemia, cortisol and prolactin levels) varied less with nerve blockades than with general anesthesia, but after nerve blockade performing prolactin increased four-fold and correlated with situative anxiety. Postoperatively, pain intensity and need of opioids were significantly less and analgesia duration was longer with nerve blockades. An algorithm of the best approach selection for the brachial plexus block was proposed.
