Aim: to study the experience of diagnosis and treatment of thyroid and adrenal tumors and develop a modern program of their surgical treatment for reducing the risk of treatment, the number of relapses and postoperative complications. Methods: clinical, epidemiological, morphological, biochemical, statistical. Results: a new complex strategy to perform thyroid surgery based on anatomical and topographical variations of prevalence of pathological process was developed and applied. The algorithm of data analysis from an electronic registry of patients with focal thyroid pathology for prolonged observation of benign focal neoplasms to optimize the diagnostic and treatment approaches in each concrete clinical situation was developed. Novelty: The data obtained allow to reduce the duration of surgery and general anesthesia to 1.5 times, the number of administered analgesics in the postoperative period, the incidence of laryngeal complications and persistent parathyroid disease, the patient's stay in hospital by 2.6 times. Advantages over analogues: The difference of the study is in the absence of relapses in 97.7 % of patients who underwent to organ surgery for papillar or follicular thyroid carcinoma. This demonstrates the possibility to perform operations providing a balanced individual approach to each concrete situation.
