Aim of the study: improve the methods of preoperative diagnosis and postoperative clinical follow-up regarding an increase in life expectancy for patients of young age with thyroid cancer. Methods of investigation: ultrasound, cytological, radiological, roentgenologic, cardiologic, laboratory methods. Results: a method was developed, to calculate correction factors of measurement of thyroid volume for one group of patients with known correction factor and test device. Radioiodine-negative metastases detected in postoperative period were shown to differ from radioiodine-sensitive metastases of papillary thyroid cancer in percentage of thyrocytes that express thyroid peroxidase. An assessment of the asymmetry of thyrocyte nuclei using computer morphometric analysis may be used for differential diagnosis of papillary carcinomas, follicular adenomas, and nodular goiters. Courses of radioiodine therapy and combined therapy (surgery and suppressive hormone therapy) allowed to obtain a positive outcome in 74.2 – 86.0% of young subjects with thyroid cancer. Different fluctuations of thyroid status of females after thyroidectomy and radioiodine therapy represent a risk factor of development of hyperplastic processes, including genital endometriosis, glandular hyperplasia of endometrium, hysteromyoma, dyshormonal hyperplasia of mammary glands. Theoretical and practical novelty: the data obtained represent new and important information which will be taken into account in further studies on the identification of radioiodine-negative metastases in postoperative period. Novelty and advantages over analogues: the present study differs from current ones in an assessment of asymmetry of thyrocyte nuclei using computer morphometric analysis, which will be used for differential diagnosis of benign and malignant tumors. Efficacy of introduction: the data will be used to improve preoperative diagnosis and radioiodine therapy of metastases. Sphere of application: endocrinology, oncology, radiology.
