The preconditions and prospects of clinical and pharmaceutical service creation at inpatient hospitals and pharmacies of Ukraine are grounded. So is the role and economical efficacy of clinical pharmacist's professional activity as an insurance medical/pharmaceutical expert in Ukraine. The clinical and pharmaceutical constituent of rational pharmacotherapy concept model in Ukraine's healthcare system is scientifically grounded and developed. The model of clinical pharmacist's activity in pharmacovigilance system is worked up. The methodology of local formulary creation for the separate healthcare establishments is developed and approved. The methodology of evidence-based search of information about medications in daily clinical pharmacist's professional activity is developed. The method of therapeutic exchange of the synthetic drug in favor of herbal medicine is approved from the viewpoint of safety of pharmacotherapy enhancement. The principal approaches to drug-related problems solution are grounded and developed for the first time. Their effect on efficacy, safety and quality of multiple sclerosis, metabolic syndrome, and rheumatoid arthritis pharmacotherapy is determined. The main constituents of the pharmaceutical care focused on the physician, nursing personnel and patient in gynecological, neonatological, ophthalmological, metabolic syndrome pharmacotherapy process and phytotherapy are for the first time designated. The model of clinical pharmacist's professional activity at family practitioner's district area is at first time created. On the behalf of the results of clinical economic and pharmacoeconomic analysis of acute cerebrovascular pathology, ischemic stroke, out-of-hospital pneumonia, metabolic syndrome and antimicrobial drugs application in patients with combined trauma, the possibility of costs for pharmacotherapy optimization is shown, hence its rationality with adaptation to patient's social-economic status and the real abilities of the healthcare system.
