G. Yanovsky NAMS of Ukraine" popup.description2 Object - 120 patients with pulmonary tuberculosis. Aim - to develop an algorithm of surgical care for patients with MDR pulmonary tuberculosis. Methods - clinical, surgical, radiological, laboratory. The algorithm of surgical treatment of patients with MDR TB, based on the characteristics of the disease affecting the outcome of treatment has been developed, that involves certain terms and extent of investigations and pre-operative sequence to determine the type and technique of surgical intervention and technology postoperative treatment. It was established that the use of the algorithm achieves cure rates of 97.6 % of patients in the absence of mortality (in the comparison group - respectively, 91.7 % and 2.5 %). It was established that in the absence of surgical treatment of patients with MDR tuberculosis with existing indications for surgery in up to 18 months of treatment its performance deteriorates to 25.0 % due to treatment failure (20,0 %) interrupted treatment (30,8 %) of complications and mortality (16,7 %). A method of dissection of the soft tissues in the performance of thoracotomy, the use of which allows to reduce by half the length of surgical access (with 27,5 min to 14,5 min) and reach the absence of postoperative complications in the wound.
