Object of study: pain syndromes of the lumbar spine caused by degenerative and traumatic lesions. Objective: improvement of treatment outcomes of patients of all ages with pain in the lumbar spine with intervertebral disc protrusions in the lumbar spine and vertebral-spinal trauma on the basis of the differentiated use of minimally invasive surgical techniques. Methods: clinical and neurological, traditional X-ray (spondylography with functional tests); neuroimaging (computed and magnetic resonance tomography of lumbar spine); neurophysiological (electroneuromiography); statistical. The correlation of clinical manifestations in patients with pain in the lumbar spine, caused by the protrusion of the intervertebral discs, depending on the results of neuroimaging methods have been studied. Scientific data on immediate, intermediate and long-term results of treatment of patients with pain in the lumbar spine using needle techniques have been supplemented, their comparative assessment have been performed. A methodology of the differentiated use of various minimally invasive transcutaneous treatment (prolonged epidural pharmacotherapy and needle laser microdiscectomy) in patients with protrusion of intervertebral discs in the lumbar spine have been scientifically proved, developed and implemented, depending on the degree of hydrophilicity of gelatinous nucleus. Indications have been optimized and microsurgical technique of re-intervention have been improved in patients with discogenic pain syndromes in the lumbar spine by applying differentiated surgical approach. Diagnostic algorithm have been developed and differential indications for surgical treatment of patients with pain caused by uncomplicated trauma of lower thoracic and lumbar spine have been optimized.
