Purpose - to increase efficiency and improve immediate and long-term outcomes of patients with malignant brain tumors with the use of prolonged intraarterial administration of chemotherapy and artificial hyperglycemia. Methods - standard set of clinical and instrumental, angiography, axial computed tomography, magnetic resonance computed tomography, single photon emission computed tomography, superselective endovascular method of delivering chemotherapy using a balloon catheter. Method prolonged intraarterial administration of chemotherapy. Scheme artificial hyperglycemia. Biochemical methods: determination of the content of sulfhydryl groups, total protein, protein fractions, fibrinogen, AST and ALT activity in plasma of patients. The obtained results - found that the size, location, nature of the blood supply to the tumor, severity of hypertension, compression and dislocation syndrome causes the tactics of treatment of malignant brain tumors. The technique of prolonged intraarterial administration of chemotherapy. Using selective delivery of drugs to the tumor and reduce their generally toxic action by intraarterial infusion enhances antitumor effect of chemotherapy. Positive results of treatment was observed in 48 patients, accounting for 83.4% of treated patients. According to clinical data (improving quality of life) conventional intra chemotherapy was effective in 77.8% of patients, whereas chemotherapy using the method of prolonged intraarterial administration of chemotherapy combined with artificial hyperglycemia was effective in 86.6% of patients with malignant brain tumors. The number of patients without recurrence for 3 years in the group with prolonged intraarterial administration of chemotherapy combined with hyperglycemia was 17.7%. Results of clinical studies show that prolonged use of the method intraarterial administration of chemotherapy combined with hyperglycemia in patients with malignant brain tumors increases the effectiveness of treatment by 13.1%. A more pronounced improvements biochemical parameters in plasma of patients obtained after prolonged treatment with intraarterial administration of chemotherapy against a background of hyperglycemia, indicating that the efficiency of the treatment.
