The purpose of research - to study the early and late results of combined endovascular treatment of patients with ischemic brain lesions and to develop measures to improve the effectiveness of endovascular interventions in these patients. Methods: Standard survey, neurological status, cerebral angiography (CAG), computer and magnetic resonance imaging, duplex scanning of vessels, electrocardiography, echocardiography, laboratory parameters. Results: The analysis of invasive and noninvasive diagnostic methods stenotic lesions of cerebral vessels showed that the CAG data were decisive for endovascular operations. Analysis of early and late results of endovascular treatment of patients with ischemic brain lesions showed that in the early postoperative period in the first group of hemorrhagic complications was not, ischemic complications reported in 7.5% of patients were transient in nature. In group II complications were not recorded, and the improvement of cerebral hemodynamics found in 84.2% of patients. Effect pharmaco infusion depended on the stage of cerebral circulation. Analysis of long-term results showed that endovascular surgery have preventive value: among patients who were under the supervision of dynamic systems, no new cases of stroke. Transient ischemic attacks occurred only in 6.4% of patients did not lead to a stroke or deaths. Good and satisfactory situation prevailed in almost 98.4% of patients within 6 months and 87.3% of patients 3 years after surgery. In general, for 3 years there have been no cases of ischemic or hemorrhagic stroke, or death due to stroke. Only 3 patients reported asymptomatic restenosis. The factors influencing the prognosis of endovascular treatment of patients with ischemic stroke. Developed tactics combined endovascular treatment of such patients in different periods of the disease (holding pharmaco infusion, angioplasty, thrombolysis, stenting). Improved technique of surgical interventions with stenotic-occlusive lesions of cerebral vessels. The features of endovascular interventions on vessels of various localization: in the first place, on vertebral and intracranial vessels.
