Objective: To increase the effectiveness of treatment of patients with uterine sarcomas by its individualization through a comprehensive evaluation of malignant potential tumors based on the immune-histochemical studies. Methods and apparatus: roentgenological, morphological, clinical, instrumental, statistical; universal dosimeter UNIDOS, sledge microtome MS-2, light microscopes. Theoretical and practical results: Molecular markers were studied in uterine sarcomas and the peculiarities of tumor metastasis and recurrence according to marker level were determined. The value of Bcl2 as positive prognostic factor both in leiomyosarcoma and endometrial stromal sarcoma was proved, especially on the background of p53 negative expression. A conclusion was reached that the presence of progesterone receptors is also a positive predictor for the survival rate. It was established that the disease progression incidence in leiomyosarcoma is 43.9 %, including local tumor recurrence, which was observed in 22.2 % of patients, distant metastases - in 77.8 %. For all disease stages combined the 5-year non-progressive survival rate was 56.1 %. For endometrial stromal sarcoma cases this index was 48.5 %, including distant metastases - in 68.8 % of cases, while local tumor recurrence was observed in 31.3 % patients. The overall 5-year non-progressive survival rate for all stages combined appeared to be 54.5 %. There were no specific peculiarities in metastasis process depending on tumor histological structure. Novelty: The peculiarities of expression and impact of the panel of immunohistochemical markers (p53, Ki-67, Bcl 2, ER, PR, etc.) on clinical and morphological characteristics of tumors with different histological forms of uterine sarcoma were studied for the first time. The informational value of the prognostic criteria was assessed. The effectiveness of implementation of the results: іn the absence of markers of Bcl 2 and p53 in the tumor treatment of inappropriate conduct further, which leads to reduction of treatment cost by eliminating chemotherapy, reduced hospital stay days.
