Purpose - to improve outcomes in patients with cancer of the head and neck through the development of individualized approaches to the selection of neck dissection volume. Research methods and instruments: clinical, clinical laboratory, radiological, pathological, statistical, single-photon emission computed tomography "OFET", ultrasound. Individualized approaches to the selection of neck dissection volume in patients with head and neck cancer were developed. It was found that sentinel lymph node identification makes less traumatic functional-saving selective neck dissection possible, their volume was determined by localization of "sentinel" lymph node in the I-III levels. Conducting neck dissection with intraoperative detection of "sentinel" lymph node allowed to hold more than half (52.8%) patients with functional-preserving selective neck dissection. A new method of performing selective neck dissection II, III and ІVA lymph nodes levels whith preserving the blood supply and innervation of the sternocleidomastoid muscle.
