M.I.Sitenko popup.description2 Purpose of the study - to improve the results of surgical treatment of idiopathic scoliosis by determining the criteria for selecting the method of instrumented ventral spinal fusion. Methods - information and analysis, X-ray, MRI, CT, experimental biological modeling, biomechanical, mathematical modeling, clinical, statistical analysis. Meta-analysis of scientific literature, experimental biological modeling, experimental biomechanical studies and mathematical modeling were performed in which criteria for selecting the method ventral spinal fusion to correct scoliosis were developed. Based on criteria developed a new method of corrective ventral spinal fusion and method for determining structural component of spinal deformity proposed. It was found that the use of ventral and posterior spinal fusion result in restriction of mobility of the spine compared with healthy individuals, but the use of ventral instrumentation of the spine in scoliosis is preferred by 60% than the posterior in terms of maintaining a greater degree of mobility of the spine compared with healthy individuals. For the purpose of verification criteria developed by a comparative analysis of the results of the new method with the results of the use of ventral and selective posterior spinal fusion in 36 patients with idiopathic scoliosis. The study found that compared to posterior spinal fusion, the use of ventral fusion reduces blood loss, postoperative pain intensity, patient's hospital stay, and improves the quality of life of patients in the postoperative period separated. PSF compared with VSF provides better by 17% correction. At the same time, use of both surgical methods results in identical treatment and cosmetic result of the dynamics of change of symmetry of the body within 2 years after surgery.
