Aim of the research - to improve the results of total hip arthoplasty. Methods of research - clinical, roentgenological, roentgendensitometry (measurements of optical density of bone tissue with program "X-ray"), dualenergy x-ray absorciometry, mathematic modelling (finite element method), statistical. Early and mid -term results of total hip arthroplasty were studied with the usage of different stems of fixation. New data were obtained about changes of bone mineral density around femoral stems at different shapes of femoral channels. Due to mathematic modeling the peculiarities of strain distribution in the system "femur-stem" were revealed at the different cooperation of femoral canal and type of stem fixation. Calculations showed that strains distribution depended on the shape of femoral canal and applied stem. On the base of obtained results the method of stem choice was developed. The indications for the usage of different stem designs and types of fixation were specified in patients with conical, cylindrical and normal shapes of femoral channel. Observance of the developed rules of selection of a femoral component in each case allows to increase quality of treatment, to decrease the frequency failed arthoplasty on the reason of aseptic loosening or stress-shielding appearance.
