Objective: to develop a technology for early diagnosis of the alimentary canal diseases in its upper divisions in adolescents considering HP toxygenicity. Methods: clinical, biochemical, instrumental, ELISA, histological, immunohistochemical, and a respiratory test (sensory analysis). Results: the frequency and clinico-morphofunctional features of CagA-associated functional dyspepsia, chronic gastroduodenitis and ulcerous diseases have been established in the examined adolescents. Negative effect of CagA strains of HP on gastroduodenal mucosa has been proved, resulting in the development of more severe variants of its lesions and a more pronounced clinical symptomatology of the disease. It has been revealed that CagA-associated gastroduodenal pathology in adolescents is accompanied by a decrease in protective mechanisms, including daily melatonin excretion mainly in boys, and some changes in gastric mucin formation (from its activation in the functional changes to a sharp drop in the presence of destructive changes with domination of epitheliocytes death process over the proliferation process, according to the findings of Ki-67 proliferation marker). Our study has first implemented the idea of recognition CagA strains of HP according to the products of human breathing, using home-produced sensor devices, and the exhaled gas properties, depending on the CagA of HP infection, have been identified. The diagnostic algorithm and technology for medical support of adolescents with digestive diseases of H. pylori etiology have been worked out. Novelty: early diagnostic criteria have been first established on the basis of computed informativeness of functional, chronic inflammatory and destructive pathology of the gaster and duodenum in adolescents, infected with CagA of HP. The diagnostic algorithm for early detection of CagA-associated gastroduodenal pathology has also been developed. Our study has demonstrated that melatonin excretion level and the presence of CagA of HP infection are important criteria that can be used by clinical practitioners in medical health care institutions to improve follow-up observation of adolescents with HP-associated gastric dyspepsia. Effectiveness: introduction of the research outcomes will provide timely diagnosis of CagA-associated gastroduodenal diseases, improve the efficiency of treatment and the results of regular medical clinical observation.
