Objective: to deepen the existing notions regarding mechanisms of AH formation in obese adolescents. Methods: clinical, biochemical, radioisotope, enzymatic, instrumental, and statistical. Results: the elevated frequency and depth of endothelial dysfunction have been observed in adolescents with AH and an increased body mass index, which correlates closely with the disorders in the blood lipid profile, carbohydrate and purine metabolism and is accompanied by a raise in angiotensin-II and aldosterone levels and in a number of factors of systemic inflammation, some changes in the NO metabolites and endothelin ratio, microalbuminuria, as well as biventricular cardiac remodeling. There have been singled out some components that are most important in the formation of endothelial dysfunction in adolescents with AH, overweight or obesity, namely: factors of subclinical inflammation (C-reactive protein and TNF), lipid and purine metabolism disorders as well as IR. Novelty: hypertension in adolescents with obesity is accompanied by accumulation of aggressive both corrected and non-corrected risk factors, high frequency of metabolic disorders (blood lipid spectrum, insulin resistance, hyperuricemia) that arise against the high activity of the renin-angiotensin-aldosterone system, endothelial factors imbalance and systemic inflammation factors as well as create conditions for the development of endothelial dysfunction and remodeling of target organs (the heart and kidneys). Effectiveness: a method for diagnosing endothelial dysfunction in adolescents with AH and obesity has been worked out. Application field: pediatric cardiorheumatology, pediatrics.
