Objective: to improve preventive methods for the severe course of the type 1 DM in children and adolescents. Methods: clinical, biochemical, psychodiagnostic, questionnaires, and statistical. Results: unsatisfactory level of self-control has been revealed in 70% of our patients that is closely associated with metabolic decompensation and severe course of the disease. The frequency of diabetic complications depends on the state of self-control and is higher in the group with its insufficient level. A decreased frequency both of lipodystrophy (from 62.0% to 36.5%) and diabetic polyneuropathy (from 65.0% to 53.1%) has been registered after acquiring methods of self-control. The analysis of IPH data has shown that among structural and sense components in adolescents with type 1 DM, both before and after instruction, first three ranks take self-estimation, behavioral and cognitive-declarative components. In the group of children with optimal glycemic control the dominative components in the structure of IPH after mastering self-control are: cognitive-declarative, control, behavioral, value, and motivational- demanding, as well as the component of confidence and hope of recovery. A decreased percentage of cognitive-declarative and an increased value of emotional components have been observed in the group of patients with a high-risk glycemic control. Instruction of adolescents with DM in the School of self-control (SSC) leads to an increase in the proportion of children, who disclose IPH touching upon the pole of health only. Analysis of the parental attitude to the disease of their children has shown that after training in the SSC their anxiety and general tension decrease, at the same time understanding that the course and treatment of the disease depend on their own efforts gets clear. Novelty: the data obtained show the state of DM self-control in children in present-day conditions. A close connection between the level of self-control, the state of metabolic compensation, and formation of diabetic complications has been proved in the study. The features of IPH in our patients and the attitude of parents to the health of their children, as well as positive dynamics of these findings in the process of learning in SCC have been established. Effectiveness: learning in SCC resulted in improvement of the control level in terms of biomedical knowledge and practical skills, which is accompanied by positive dynamics of the disease compensation parameters and reduced average values of HbA1c (from 9.80% to 8.14%) due to a decreased number of patients with a high risk of glycemic control (from 67.5% to 29.3%), an increase in the frequency of optimal glycemic control (from 12.5% to 41.6%), and some changes in the psychological status of our patients.
