Objective: optimization of the diagnostics, surgical and postoperative treatment strategy for papillary cancer by studying its clinical and pathological features at the patients who were operated at the clinic of "IPEP NAMS". Research methods: general clinical, HRP, thyroid ultrasonography, histological, immunohistochemical methods of research of thyroid tissue, methods of formal genetics, statistics. It is proved that T1796A BRAF point gene mutation at the patients with PTC may be a marker of distant metastasis. A new method of the urgent histological examination for thyroid microcarcinomas was developed to optimize the choice of surgical tactics. It is determined that PTC was joined with colloidal goiter, follicular tumors, autoimmune thyroiditis, diffuse toxic goiter, that makes it so difficult to diagnose. Treatment algorithm for PTC, which was used in the past - organ-saving surgical treatment, following radiotherapy and application of thyroid preparations, proved high efficiency (99, 4 %).
