The role of ultrasonographic and videoendoscopic technologies in diagnostics and dynamics of processing of main surgical diseases and treatment policy in age terms and in polymorbidity conditions were researched. It was shown that the initiator of multiple organ failure in patients with acute peritonitis is the dysfunction of cardiovascular pathology, as evidenced by the growth of 6.4 times of the activity of necrotic processes in cardiomyocytes . The applying of effective methods of cardioprotection significantly improves the results of surgical treatment of these patients. To optimize the miniinvasive approaches for drainage of purulent parapancreatitis took into account the topographic anatomical features using sonographic (ultrasound) criteria. The use of miniinvasive multistage method under ultrasound, laparoscopy control, CAT reduced the postoperative mortality rate in complicated necrotic pancreatitis from 30.3 % to 16.6 %.
