Subject of research - clinical, structural and functional, vegetative, biochemical changes in patients with CTD and drug correction of them. Purpose - to improve outcomes in patients with CTD based on the study of structural and functional state of the cardiovascular system, endothelial dysfunction, the activity of immune inflammation, vegetative status and psychological changes and to develop differentiated treatment programs for this category of patients. The pathology of the musculoskeletal system and connective tissue is the extremely important theoretical, medical and social problem in our society. Therefore, the development of complex laboratory tests for the early detection and timely administration of DMARD, aimed to stop the progression of the disease is very actual. On the basis of summarizing the results of the studying of metabolic risk factors for cardiovascular disease in patients with RA it was established certain association with the progress of the underlying disease and morpho-functional state of the cardiovascular system. It was found that patients with RA inflammatory pattern, hyperhomocysteinemia are associated with more expressive structural and functional disorders of the vascular wall, in particular, with a significant deterioration of endothelial function and thickening of intima-media of common carotid artery. It was found that in patients with RA endomorphic somatotype is a predictor of resistance to standard therapy. The features of melatonin and cytokine profile depending on the clinical and immunological parameters of activity and psychological status were studied in RA patients and it was established the clinical and pathogenetic significance of circadian melatonin production and related cytokinemia. Analysis of results of the levels of proinflammatory and anti-inflammatory markers of inflammation, their ratio, a pathogenetic role in development of pain syndrome in FM patients showed that high levels of IL-8, in particular, the growth ratio of IL-8 / IL-10 is associated with the intensity of pain. In patients with FM based on a comprehensive study of macro- and microcirculation it was found signs of formation of endothelial dysfunction (impaired endothelium-dependent and endothelium-independent vasodilation, reduction of endothelin-1 in blood plasma), a local reduction in blood supply of the specific and sensitive points and was substantiated the approach to differential pharmacological correction autonomic and endothelial dysfunction in patients with FM. In patients with systemic sclerosis the functional state of the respiratory muscles was assessed and the presence of respiratory muscle dysfunction in the form of weakness and fatigue of the respiratory muscles was established.
