Aim: to determine the optimal chemotherapy regimens for patients with MDR TB depending on the number, amount of anti-TB drugs at the treatment regimen, the length of intensive and continuation phases of chemotherapy, which will increase the treatment efficasy and save public funds. Methods: clinical, radiological, laboratory, microbiological. Developed by step therapy of linezolid at a dose of 0,6 g 2 times to the sputum conversion, the transition at a dose of 0,6 g 1 time per day to the completion of chemotherapy, which led to a reduction in terms of sputum conversion on 3 weeks and to reduce the cost-efficiency on 1,8 times. Developed the method of use glutamyl-cysteine-glycine disodium to improve the tolerability of chemotherapy, which led to an increase the frequency sputum negatev by 11,5 % and the disappearance of clinical and radiological manifestations of the disease in 14,2 %, and reduced the frequency of adverse event on 28,5 %. Individualized regimens for patients with XDR TB were developed: additional use of etionamid to 5 anti-TB drugs I-II lines, compared with regimes which used only 5 anti-TB drugs I-II lines without etionamid resulting to received sputum and culture conversion in 89,9% of patients, and to increase the treatment efficacy of 18,9 %, and was reduced the cost-effectiveness of chemotherapy regimen in patients with 402,4 to 322,6 UAH.
