Objective: To determine the relationship between the functional and structural disorders of the liver, immune status against the background of formation and progression of the portal hypertension in patients with chronic diffuse liver diseases. Methods: clinical, instrumental, morphological, biochemical, immunological, immunoenzymometric, pathophysiological, statistical. Progression of portal hypertension leads to the replacement of liver parenchyma tissue, reduction of the sinusoid diameter, increasing signs of encephalopathy. Cytolytic syndrome was typical for all patients. Depressive type immune response has been observed with simultaneous increase in circulating immune complexes. Scientific novelty: determining the degree of liver fibrosis according to the sonic (Shear Wear) and mechanical (FibroScan) elasticity imaging in comparison to morphological study of liver parenchyma. Theoretical significance: obtained data allowed examining the actual problem of modern clinical medicine and deepening knowledge about the formation and progression of portal hypertension in patients with chronic diffuse liver disease. Practical significance of the work is in the distribution of the portal hypertension severity based on liver parenchyma hardness according to shear wave elasticity imaging. The results of research implemented in the method of determining the degree of liver fibrosis based on identifying specific biomarkers and their correlation coefficients.
