Informative criteria of early signs of disease were identified. The study established the presence of obstructive ventilation disorders not only according to spirometry, but also bodyplethysmography, as evidenced by growth of static lung volume rates. Proved, that the decrease rate of the diffusion capacity of the alveolar capillary membrane (DLCO) is the result of the excessive development of fibrous tissue in the lungs. Acoustic breath sounds in miners with COPD using phonospyrographic complex "KoRA-03M1" were studied, and objective criteria for pathological symptoms of COPD were identified. The role of viral infections in exacerbations of occupational COPD also was proved. The presence of enteroviruses with stable course of disease shows their role in triggering exacerbations of COPD. The role of genetic component in the development of COPD among coal miners was proved. According to a coefficient of family aggregation in miners with a family history of bronchopulmonary pathology. COPD risk in 2.8 times higher than in the general population. The fact, that genotype of MMP-9*TT is a marker of genetic susceptibility to COPD and genotypes TIMP-2*GG and MMP-9*SS; TIMP-2*G/G are markers of resistance to COPD in miners of coal mines, was proved by the method of polymerase chain reaction. The recommendations on improving of diagnosis system and prevention of COPD in workers under the influence of coal dust were developed.
